Apply

If you are receiving Wajong (young disabled persons benefit) and you apply for conversion of your performance grant, you do not need to get in touch with your student counsellor or study advisor. In all other situations, the application for a performance grant provision should be made via your student counsellor or study advisor.

Any application needs to include 2 supporting documents: a medical certificate and a statement from the educational institution.

Conversion performance grant if you receive Wajong

1. Complete the ‘Aanvraag voorziening prestatiebeurs bij Wajong’ form.
2. Attach the supporting documents requested in the notes on the form.
3. Send the application to the address stated on the form.

Other applications

1. Get in touch with your student counsellor or study advisor as soon as possible. In exceptional situations, the counsellor or advisor can apply for a performance grant provision on your behalf. The student counsellor or study advisor has the application form.
2. Complete question 1 (your details). The student counsellor or study advisor completes question 2 (application for provision). The application needs to be signed by both of you.
3. Attach the supporting documents requested in the notes on the form. Send the application to the address stated on the form.
